LIMITED – A company making plastic containers discovered
that a rising percentage of its output has developing cracks. A
project group studied the manufacturing process in detail and
eventually defined the point of deviation: a change of supplier
and failure to clean out storage vats. The new supplier
inadvertently used storage vats and the plastic therefore became
contaminated. Once the causes had been identified, the systems
were altered to prevent any repeat , and the problem did not
recur.
